Job Overview
The Part-Time Case Manager plays a critical role in The Peoples Action’s Community Violence Intervention (CVI) program by providing individualized case management services to justice-impacted and high-risk participants. This role focuses on stabilizing participants, addressing immediate needs, and supporting long-term goals related to safety, wellness, education, employment, and compliance with court or supervision requirements.
The Case Manager works closely with street outreach workers and partner agencies to ensure coordinated, trauma-informed, and culturally responsive service delivery.
Key ResponsibilitiesParticipant Engagement & Case Management
- Conduct participant intakes, assessments, and service planning in collaboration with the CVI team
- Develop individualized service plans based on participant needs, risks, and goals
- Support participants with referrals and warm handoffs to services including housing, employment, education, behavioral health, substance use treatment, and legal resources
- Assist participants in navigating systems such as probation/parole, courts, schools, and social service agencies
Coordination with CVI Team
- Collaborate closely with street outreach workers to support high-risk participants
- Participate in case conferencing, safety planning, and multidisciplinary team meetings
- Share relevant updates with the team while maintaining confidentiality and professional boundaries
- Support mediation and intervention efforts as appropriate, in coordination with outreach staff
Documentation & Data Management
- Complete timely and accurate documentation including intakes, case notes, service referrals, and progress updates
- Use program data systems and tools to track participant engagement and outcomes
- Ensure documentation meets internal, funder, and compliance requirements
Community & Partner Engagement
- Build and maintain relationships with community-based organizations, service providers, and institutional partners
- Advocate on behalf of participants to reduce barriers to services
- Represent The Peoples Action professionally in community and partner settings
Required Qualifications
- Associate’s or Bachelor’s degree in Social Work, Human Services, Criminal Justice, Psychology, or a related field (or equivalent lived/professional experience)
- At least 1 year of experience in case management, social services, outreach, or a related role
- Experience working with justice-impacted individuals, high-risk populations, or community-based programs
- Strong understanding of trauma-informed, harm reduction, and strengths-based approaches
- Ability to work independently and collaboratively in fast-paced, community-based environments
- Strong documentation, organization, and communication skills
Preferred Qualifications
- Bachelor’s degree in Social Work (BSW) or related field
- Experience in a Community Violence Intervention, reentry, or public health setting
- Familiarity with Detroit-area resources and service networks
- Experience working with individuals on probation or parole
Work Environment & Expectations
- This position involves field-based work in community settings
- Flexibility in schedule, including occasional evenings or weekends, may be required
- Must be able to travel locally for participant meetings and partner coordination

Pay: $28.12 per hour
