Overview
We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our dynamic care team at a reputable Skilled Nursing Facility in the Rockaway Park, NY area. This role is ideal for a clinician passionate about delivering high-quality, patient-centered care to elderly and post-acute populations.
Day shift and no on-call or weekend requirement
Duties:
- Conduct comprehensive assessments and manage care for residents
- Diagnose and treat acute and chronic conditions within scope of practice
- Collaborate with nursing staff, rehab therapists, social workers, and physicians
- Participate in care planning, family conferences, and transitions of care
- Maintain accurate and timely documentation in the EMR
Qualifications:
- Licensed Nurse Practitioner or Physician Assistant in the state of New York
- Experience in geriatrics, long-term care, or internal medicine preferred
- Strong interpersonal and communication skills
- Ability to work independently and collaboratively in a team setting

Pay: $160,000.00 per year
