Pay Rate: $29/hourOverviewWe are seeking a detail-oriented and compassionate Clinical Assessment Coordinator II to support a statewide program focused on long-term care services. In this role, you will conduct assessments and provide intake counseling for individuals seeking Medicaid Waiver and Long-Term Services and Supports (LTSS).
This position plays a critical role in ensuring individuals are accurately assessed and connected to appropriate care resources, helping improve access to essential support services and overall quality of care.Key Responsibilities
- Conduct comprehensive clinical and functional assessments for program participants
- Provide intake counseling and education on available services and care options
- Ensure all assessments are completed in compliance with state guidelines and required timelines
- Accurately document findings and maintain detailed case records
- Coordinate with internal teams, providers, and community resources
- Manage a high volume of cases while maintaining quality and accuracy
- Support individuals transitioning between care settings
- One of the following: Registered Nurse (RN), Bachelor’s degree in Health/Social Work or related field, Associate degree in Nursing, or Master’s degree in any field
- At least 1 year of experience in human services, healthcare, or related field
- Knowledge of behavioral health diagnoses, psychotropic medications, and general medical conditions
- Strong ability to assess individual needs and provide appropriate guidance
- Excellent organizational and multitasking skills
- Proficiency in Microsoft Office (Word, Excel, Outlook)
- Strong written and verbal communication skills
- Ability to work independently in a fast-paced, remote environment
- Experience with InterRAI assessment tools
- Background in Medicaid Waiver or LTSS programs
- Knowledge of state-level healthcare regulations
- Familiarity with HIPAA compliance and person-centered care practices
- Additional certifications in healthcare, social work, or human services
- Company-provided equipment will be supplied
- Reliable high-speed internet required: minimum 20 Mbps download (50 Mbps if shared connection) and 5 Mbps upload
- Private, secure workspace free from distractions
Pay Rate: $29/hour
