Licensed Practical Nurse (LPN) – Assisted Living | The Osborn – Rye, NY
Full-Time, Part-Time & Per Diem Shifts Available
Pay: $36.18 – $38.43 per hour
The Osborn, a 5-star senior living community in Rye, NY, is seeking compassionate and skilled Licensed Practical Nurses (LPNs) to support our Assisted Living residents. Position Summary:
In our Assisted Living setting, LPNs play a critical role in maintaining residents' health, independence, and quality of life. You will work closely with residents, families, and interdisciplinary team members to ensure individualized care plans are followed and health and safety guidelines are upheld.
Responsibilities:
- Monitor residents’ conditions and assess their needs to provide the highest quality care
- Recognize and report changes in condition to physicians and RNs
- Administer medications and treatments in accordance with physician orders and facility policies
- Document care provided, symptoms, and observations accurately and promptly
- Collaborate with staff, physicians, and families to deliver coordinated, compassionate care
- Maintain compliance with safety and infection control standards
- Foster a positive, respectful, and welcoming environment for residents and their families
What We’re Looking For:
- Previous LPN experience preferred but not required
- Knowledge of Assisted Living regulations and standards is a plus
- Sound judgment and decision-making abilities in a residential care setting
- Excellent interpersonal and communication skills
- Strong organizational and multitasking abilities
- Valid NYS LPN license
