The Olympia Free Clinic (TOFC) is a volunteer-based, 501(c)(3) nonprofit organization that provides medical, mental health, and specialty care services to uninsured and underinsured adults at no cost to the patient. We provide accessible, high-quality health services in a caring, respectful environment, and work to connect our patients to other community resources in pursuit of improved overall health.

We are a vital resource for a diverse and rapidly growing patient population. We center equity in our work by bridging gaps in care to meet patients’ unique medical needs. In 2025, our clinic provided over 2,300 visits to 723 individual patients.

About the Position

TOFC is hiring a part-time social worker. This is an exciting opportunity to design, build, and strengthen a new program for the organization. This position plays a vital role in ensuring that every patient who walks through The Olympia Free Clinic’s doors receives compassionate, coordinated, and culturally responsive support. The position works directly with uninsured and underinsured adults who often face complex medical, social, and systemic barriers. As a core member of our care team, this position meets with patients during clinic hours to assess needs, provide brief interventions, and connect individuals to community resources that promote stability, health, and long‑term well‑being.

This role bridges the gap between medical care and the social determinants that shape health outcomes. The social worker/social work coordinator supports patients with tasks such as navigating follow‑up plans, understanding referrals, addressing transportation challenges, completing forms and applications, and identifying next steps after receiving care at the clinic. The position also collaborates closely with TOFC’s volunteer medical and mental health providers, and community partners to ensure continuity of care and reduce preventable crises.

Essential Duties and Responsibilities

Depending on candidate’s qualifications and credentials, duties and responsibilities may include:

Assessments and Interventions:

  • Conduct assessments to identify client needs and assist clients in accessing appropriate social services including health care, housing, and mental health resources. Examples may include cognitive assessments for dementia, depression, anxiety; and assessing patient access to preventive resources.
  • Provide individual and family counseling or crisis intervention as needed; as needed, design and facilitate group support sessions.
  • Use motivational interviewing and solution-focused therapy around chronic disease self-management and adjustment to diagnosis.
  • Lead basic health education for patients and caregivers.
  • Assist patients to apply for medication assistance programs.

Case Management:

  • Manage multiple client cases, develop treatment plans, and monitor progress to ensure effective service delivery.

Identify Resources and Referrals:

  • Identify common health education and resource needs in TOFC’s patient population.
  • Connect and create relationships with outside resources.
  • Create a library of patient handouts for education around chronic conditions, medication management, and other key topics.
  • Build relationships and referral systems with outside organizations.

Program Development, Data Collection, and Evaluation:

  • Design and implement data collection mechanisms, including a brief assessment for patients to complete with new patient registration.
  • Design and implement organizational policies and procedures for the social work program.
  • Design an in-house mechanism for TOFC volunteer providers to refer patients to the social work program.

Documentation:

  • Maintain accurate EHR records of client interactions, treatment plans, and progress notes in compliance with legal and ethical standards.

Advocacy:

  • Advocate for clients' rights and needs within the community and help them navigate complex social service systems.

Required Qualifications and Experience

  • BSW or MSW.
  • LSW / LMSW; LCSW / LICSW; or LSW‑Associate.
  • Applicants must pass a background check before hire.
  • Employee must be proficient in use of computers, the internet, and be comfortable learning and adopting new technologies. To perform this job successfully, an individual should have knowledge of: MS Office (Outlook, Teams, Word, Excel, PowerPoint, Forms, OneDrive); Google Suite (Google Drive, Docs, Sheets, Forms); Spreadsheets (Excel, Google Sheets, csv files); Web-based applications (e.g. virtual phone system manager, electronic fax and messaging system, etc.); Experience with Electronic Health Records.

Volunteer, internship, or paid experience supporting:

o Low‑income individuals and families — demonstrated ability to provide resources, advocacy, and services to clients experiencing economic hardship.

o Persons experiencing houselessness — skilled in trauma‑informed, person‑centered engagement with individuals navigating unstable or temporary housing.

o Individuals with mental or behavioral health needs — comfortable collaborating with clients living with mental illness, behavioral challenges, or co‑occurring conditions.

o Multicultural and multilingual communities — ability to work effectively across diverse cultural backgrounds, languages, and communication styles.

o Diverse gender identities and expressions —strong commitment to gender‑affirming, inclusive practices for trans and non‑binary individuals.

Desired Qualifications and Experience

  • Spanish proficiency or fluency.
  • Experience working in a free clinic or nonprofit organization.
  • Experience working with volunteers/in a volunteer-based setting.

FTE Classification: .375 FTE nonexempt (15 hours per week)

Anticipated Schedule: The successful candidate must be available in-person for one clinic night per week (Monday or Wednesday, approximately 4 p.m. - 9 p.m.), Friday daytime clinic (10 a.m. - 2 p.m.), and 5-6 non-clinical hours for program development, which can be remote.

Benefits: This position is eligible for paid sick leave.

Reports to: Executive Director.

Clinical Supervision: Depending on qualifications, clinical supervision provided by a TOFC licensed social work volunteer.

Start Date: August 1, 2026
