Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, skilled nursing services across in long-term care facility.
Duties
- Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions to inform care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care.
- Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
- Perform emergency interventions such as CPR and first aid when necessary.
- Document all patient interactions meticulously within Electronic Health Records (EHR) systems in PCC.
- Coordinate with interdisciplinary teams to develop and implement individualized care plans for patients with complex needs such as dementia, Alzheimer's disease, or developmental disabilities.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in your jurisdiction.
- Proven experience working in diverse clinical environments such as long-term care facilities, hospitals (including ICU or trauma centers), emergency rooms, or home health settings.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or eClinicalWorks; familiarity with ICD coding (ICD-9/10) is highly desirable.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology essential for accurate patient assessment and documentation.
- Experience administering injections, performing venipuncture (phlebotomy), catheterization, tube feeding management, and ventilator support is preferred.
- Excellent communication skills for effective caregiver collaboration and client interaction; experience working with geriatric patients or individuals with disabilities is a plus.
- Ability to work independently in high-pressure situations such as acute care or emergency medicine environments while adhering to safety protocols like sterile technique and infection control standards.
