Job Summary: We are a fast-growing private behavioral health practice seeking a Targeted Case Manager (TCM) to join our supportive, client-centered team. This role is ideal for someone who is passionate about helping individuals with mental health needs access the care, resources, and support they deserve.
As a TCM, you’ll provide case management services to individuals diagnosed with behavioral health conditions, assisting with goal setting, care coordination, and connection to essential community services.
Responsibilities:
- Provide community-based behavioral health case management to eligible adult clients
- Complete assessments and develop individualized service plans (ISPs)
- Coordinate services with mental health professionals, healthcare providers, and social service agencies
- Assist clients in accessing housing, education, employment, and other support services
- Assist clients in applying for and resolving lapses in KY Medicaid/MCO coverage
- Maintain accurate, timely documentation in accordance with Medicaid and state guidelines
- Monitor and evaluate client progress, adjusting plans as needed
- Participate in team meetings, supervision, and training
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related human services field (required)
- Must be eligible for or hold current Targeted Case Management (TCM) certification in Kentucky (required)
- Minimum of 1 year of TCM experience working with adults with SMI diagnoses required
- Reliable transportation and valid driver’s license (required for community-based visits)
- Strong communication, documentation, and organizational skills
- Ability to work independently and manage time effectively
