Group Home Setting (6 Medically Complex Patients)

The Nurse Network is seeking experienced, compassionate, and clinically strong RN/LPNs to join our Group Home team. This assignment involves caring for 6 medically complex individuals, requiring hands-on clinical skills including G-tube management and straight catheterization.
Job Summary The RN/LPN provides direct nursing care and oversight in a group home setting, ensuring all medical and personal care needs are met. This role requires strong clinical judgment, adherence to care plans, and the ability to support individuals with complex medical needs while promoting independence, dignity, and quality of life.
- Provide skilled nursing care for 6 medically complex patients
- Perform and manage G-tube feedings, medications, and care
- Perform straight catheterization as required
- Administer medications and treatments per physician orders
- Monitor and document patient status, changes in condition, and interventions
- Follow and implement individualized care plans
- Assist with ADLs as appropriate (bathing, dressing, toileting, mobility)
- Ensure medical equipment and supplies are maintained and functioning properly
- Maintain accurate and compliant documentation per DDS and regulatory standards
- Communicate effectively with interdisciplinary care teams
- Assist with light household and operational duties as needed
- Adapt to a fast-paced, evolving care environment
Requirements
- Active CT RN or LPN license
- Experience with G-tube care and straight catheterization REQUIRED
- Experience with medically complex patients REQUIRED
- Previous group home experience perferred
- Valid driver’s license & reliable transportation (if transporting)
- Strong clinical, communication, and documentation skills
- Compassionate, patient, and team-oriented approach
