Counselor/Case Manager - Klahre House

Worksite Location: Hood River

Work Schedule: Full-Time; 40 of hours per week; Monday – Friday; 8:00 am – 5:00 pm

Wage Range: $26.89 - $30.26 per hour

Bilingual Differential Pay: $0.50 per hour worked

Reports to: Klahre House Program Manager

Who are we looking for?

The Counselor/Case Manager develops and implements Service Plans that support program participants. They act as a “central hub” of communication and planning regarding program participants’ caseloads. They also provide group counseling to other youth.

What you will do…

• Participate in intake and placement of new clients.
• Provide group and individual counseling.
• Conduct service-based activities within the assigned program.
• Lead evidence-based treatment groups as needed.
• Complete an Assessment and Evaluation Report within the identified timeline.
• Develop and implement Individual Service and Master service Plans.
• Document services delivered and understood/follow Behavioral Rehabilitation Service
• expectations for service delivery and documentation.
• Communicate professionally with youth, family, foster families, state agencies, and any other
• partners involved in the care and treatment of youth.
• Direct foster parents on the treatment of youth in care.
• Coordinate services, activities, transportation, aftercare, etc., and communicate client progress
• and problems to parties concerned as listed above.
• Participate in psychiatric consultation and training.
• Provide oversight for all aspects of the Service Plan and applicable treatment.
• Other duties as assigned by supervisor.

Education, Experience, and Requirements…

Required:

• Bachelor’s degree in Human Services or a related field
• Valid driver’s license with auto liability insurance and reliable transportation
• 2 years of experience in the care and treatment of youth and young adults
• Successful completion of a comprehensive background check

Preferred:

• Bilingual – English and Spanish
