Responsibilities
The Fair Futures Coach is a full-time position that provides support and resources to help youth who have had experiences with the justice system, make progress towards their academic, career development, independent living, and other life goals. The Fair Futures Coach will report directly to Fair Futures Coach Supervisor. The Fair Futures Coach will be responsible for:
Coach Responsibilities:
- Provide home-based or community case management services to families to a caseload of up to 15 families, visiting families at a minimum twice a month to address family needs, decrease risk and safety factors, and strengthen families overall health and wellness
- Engage primary caregivers and other key participants throughout treatment to engage in sessions and conferences, and overcome barriers to engagement when they arise, to ensure successful goal attainment
- Complete timely and accurate documentation of session progress and collateral notes in the Care4 database systems
- Establish and maintain collaborative working relationships with colleagues, community providers, and model/external stakeholders to connect families to resources and support ongoing communication and collaboration for effective goal setting. - Responsible for engaging youth in goal setting as it relates to academic & career planning
- Participate in group, individual coaching, trainings, and all staff meetings to strengthen professional development, skill growth, & group cohesion
- Provides one-on-one and group facilitation to develop life skills training, college preparation, & leadership training to youth
- Conduct intake through the completion of initial assessments and gather essential consent forms for new cases, ensuring completeness of information and family understanding of services
