The Identity & Acceptance Case Manager's role serves as a support for SCPS's clinicians. The case manager supports the clinical teams by following up on referrals, linkage and providing resources for clients and families. They also service as a community liaison and table at community events as needed.
Salary Range: $53,500 - $57,000/year

Qualifications
Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution

Responsibilities
The Identity & Acceptance Case Manager's role serves as a support for SCPS's clinicians. The case manager supports the clinical teams by following up on referrals, linkage and providing resources for clients and families. They also service as a community liaison and table at community events as needed.
- Supports families in obtaining needed services by providing resources for families discharging from the clinic, completing and submitting referrals to connect clients to other providers, and following up on these referrals.
- Compiles and manages growing list of resources for any community referrals; behavioral health, immigration assistance, identity-based supports, social benefits assistance, schools, etc.
- Completes applications to higher levels of care including C-SPOA programs, inpatient on in-home services.
- Documents case management activity for each client in electronic health record
- Serves as community liaison; attends and represents clinic at community events, distributes brochures and information, serves as point person for any questions or inquiries.
- Coordinates with community providers, hospitals and clinics to obtain medical records, evaluations and needed documentation.
- Follows up on inquiries received through Building Futures campaign; calls and checks in with social workers and parents requesting information, walks them through services and referral process, helps screen fit for HIBH programs.
- Obtains missing insurance information for clients participating in SCPS programs; calls/reaches out to caregivers and logs details obtained.

Qualifications
- Bachelor's degree in Social Work, Psychology, Sociology or related subject area. Previous case management experience preferred, but not required.
- Knowledge of community resources across NYC
- Effective verbal and written skills
- Excellent time management and organizational skills
- Computer literacy; including Outlook, and Microsoft Applications (Word, Excel, PowerPoint, Teams)
- An understanding and commitment to complying with the New York State Central Register of Abuse and Neglect protocol (requiring staff to comply with requirements to report any suspected abuse, maltreatment or neglect)
- Related work, lived experience, or meaningful interest in mental health, child welfare, and culturally diverse populations.
- Demonstrated strong commitment to safety.
