Responsibilities

Position Description:

  • Conduct assessment including review of referral information, identify ing and engaging key participants, identifying systemic risks and protective fa ctors , and de veloping an analysis of the problem behaviors within the ecological context and model process.
  • Engage prim ary c aregiver and other key participants in a ctive change-oriented treatment by identifying and overcoming barriers to engagement and generalization of treatment.
  • Implement a problem conceptualiza tion , identify co mmunity based resources to provide family specific intervention, monitor community based intervention an d outcome review and strategy revision procedure using the model's process.
  • Maintain clear, concise and timely documentation of in tervention efforts that promote peer and supervisory review and feedba ck, and that demonstrate compliance with model requirements.
  • Maintain clear, concise and timely documentation of in tervention efforts and Family Assessment and Service Planning a ccording to the State Connection System requirements.
  • Collab orate with all relevant system s and key participants within each system to ensure their buy-in and cooperation throughout treatment.
  • Participate in all model program training, supervision and consultation activities.
  • Provide home-based interventions.
  • Report to assigned team supervisor
  • Maintain a maximum c aseload , consistent with model expectations.
  • Adhere to model expectations and implement with fidelity.
  • Follow the Key Performance Indic ators set by pr ogram and follow through with clear next steps
  • Complete Family Treatment Plans, as needed and per model expectations.
  • Weekend availability required , based on client needs.
  • Ability to travel outsid e of assigne d borough/region ba sed off of special language skill/s and client needs.
  • Ability to travel outside of assigned boroug h/region if additional support is needed.
  • Conduct intakes as needed.
  • Assist with on-c all rotation.

  • Appropriate engagement with support departments to ensure appropriate and ti mely hiring, performance management, investigation, discipline, suspension, and restructuring decisions concerning dire ct reports.

Qualifications

  • Bilingual (English/Spanish)

Education

  • Ba chelor’s Degree Human/Social Servic e related degree (to include education, psychology and administration).

Preferred Experience

  • Experience with Child Welfare
  • Experience working with families
  • Demonstrated strong commitment to safety.
