Position Summary
The New Jewish Home's Certified Home Health Agency (CHHA) is seeking dedicated and experienced Per Diem Speech-Language Pathologists (SLPs) to join our Community Services team.
In this role, the Speech-Language Pathologist will provide home-based speech therapy services to adult and geriatric patients throughout Manhattan. Responsibilities include conducting comprehensive speech, language, cognitive-communication, and swallowing assessments, developing individualized treatment plans, and providing therapeutic interventions designed to maximize patients' communication abilities, swallowing function, safety, and overall quality of life.

Essential Duties and Responsibilities
- Conduct speech-language and swallowing screenings, evaluations, and assessments as prescribed by the physician.
- Develop and implement individualized treatment plans to address speech, language, voice, cognitive-communication, and swallowing disorders.
- Provide therapeutic interventions and rehabilitative services based on each patient's clinical needs and functional goals.
- Educate patients and family members regarding treatment plans, progress, home exercise programs, and discharge recommendations.
- Prepare and maintain accurate clinical documentation, including evaluations, progress notes, treatment records, and discharge summaries.
- Monitor patient progress and modify treatment plans as appropriate.
- Collaborate with physicians, nurses, therapists, social workers, and other members of the interdisciplinary care team to ensure coordinated, patient-centered care.
- Participate in case conferences, team meetings, and care planning discussions.
- Maintain required departmental and regulatory records in a timely and accurate manner.
- Assist in maintaining therapy equipment, supplies, and work areas.
- Participate in professional development and educational activities to remain current with best practices and advancements in the field.
- Perform other duties as assigned.
Qualifications
Required
- Current, unrestricted New York State license as a Speech-Language Pathologist.
- Experience working with a geriatric population.
- Availability to provide home visits throughout the Manhattan service area.
- Strong assessment, clinical reasoning, communication, and documentation skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
Preferred
- Master's Degree in Speech-Language Pathology or a related field.
- Previous home care or community-based healthcare experience.
- Experience treating adult and geriatric patients with speech, language, cognitive, and swallowing disorders.
