The Manhattan Division of The New Jewish Home is searching for a full time MDS Coordinator RN to join their team. This position sits in our 106th , NY nursing home location.
The RAUM Manage will:
- Assume MDS responsibility for a caseload to of post-acute patients and long term residents; scheduling; completing and coordinating completion of the MDS and RAI process
- Issue Notices of Medicare Non Coverage and Detailed Notices and SNF ABNs
- Determine if patients meet Medicare coverage eligibility on admission and monitor for ongoing coverage during patient/resident stays
- Work with IDT to insure best practice & compliance in RAI process
