- Coordinate and oversee completion of comprehensive resident assessments and ongoing evaluations in accordance with regulatory requirements and organizational standards.
- Collaborate with the interdisciplinary team to gather, review, and analyze resident information to ensure accurate assessment completion.
- Ensure resident assessments accurately reflect current clinical status, functional abilities, diagnoses, care needs, and services provided.
- Facilitate the development, review, and revision of individualized resident care plans based on assessment findings and changes in resident condition.
- Monitor residents for changes in condition and collaborate with nursing leadership and care team members to ensure appropriate interventions are implemented.
- Review clinical documentation to support accuracy, completeness, and alignment with resident care needs.
- Identify opportunities to improve documentation practices, care coordination, and resident outcomes.
- Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives.
- Communicate assessment-related information and recommendations to appropriate team members to support effective resident care delivery.
- Assist with monitoring quality indicators and clinical outcomes to support continuous improvement efforts.
- Maintain current knowledge of applicable state and federal regulations, clinical standards, and assessment requirements impacting long-term care.
- Support survey readiness by ensuring assessment documentation and care planning processes are accurate, timely, and complete.
- Collaborate with nursing staff and leadership to identify opportunities for improved resident care, documentation accuracy, and regulatory compliance.
- Maintain confidentiality of resident information and ensure compliance with privacy regulations.

- Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing.
- Prior experience in long-term care/skilled nursing preferred.
- Experience with resident assessments, care planning, documentation, or clinical coordination preferred.
- Strong understanding of the physical, emotional, and psychosocial needs of the aging population.
- Excellent communication, organization, and critical-thinking skills.
- Ability to collaborate effectively with residents, families, and interdisciplinary team members.
- Commitment to providing compassionate, person-centered care.
