What You Will Do

As a Complex Care Case Manager, you will work holistically with individuals and their families to provide support, advocacy, and connection to community resources.

Qualifications

• Required: A High School Diploma or equivalent is required , combined with at least five years of experience working with families, in advocacy, or in community organizing.
• Preferred: A Bachelor's Degree in Human Services, Education, or a related field.
• At the discretion of the Executive Director, additional experience may be substituted for educational requirements.
• Knowledgeable in computer use and willing to learn new systems.
• Possess excellent verbal and written communication skills.
• Must have a safe, reliable, registered, and insured vehicle for business use.
• Ability to work in a fast-paced environment and work evenings, weekends, and holidays when necessary.
• Must successfully meet applicant background check requirements.
