Primary Responsibilities:
As the Social Services/Admissions Coordinator at our facility, your role is vital in providing social work services directly to nursing home residents and their families to enhance their social and psychological well-being. You will work as part of a multidisciplinary team to create an environment that promotes quality of life, respects cultural diversity, and empowers residents to maintain individuality, independence, and choice. Your responsibilities will encompass coordinating all aspects of admissions and discharges and managing residents social and psychological needs throughout their stay, including supporting the resident representative and/or family members.
Qualifications:
Bachelor's degree in social work or a human services field (sociology, special education, rehabilitation counseling, psychology, etc.)preferred.
Three years of supervised social work experience in a healthcare setting working directly with individuals.
