Description

Position Summary: The Community Employment & Support Specialist provides integrated support to individuals receiving both Employment Services and Community Support Services (CSS). This blended role offers on-site and off-site employment assistance, independent living skills support, and individualized goal-based services to ensure people achieve success in employment, community engagement, and independent living.

Essential Responsibilities

Employment Support (Approx. 20 hrs/week)

  • Provide exceptional employment support in accordance with each individual’s Service Agreement
  • Conduct scheduled and unscheduled check-ins at job sites to ensure workplace success and satisfaction.
  • Provide training and coaching on-site and remotely to job coaches and individuals working independently.
  • Act as point of contact for employers, building and maintaining strong business relationships.
  • Complete independent employment assessments for individuals preparing to work independently.
  • Document daily, weekly, and monthly services in compliance with state and agency requirements.
  • Ensure all employment billing units and required documentation are completed accurately.
  • Understand Department of Labor regulations and their impact on the individuals supported.
  • Assist with implementation of supported employment goals and workplace accommodations.
  • Transport individuals for employment purposes when needed.

Community Support Services (Approx. 20 hrs/week)

  • Support individuals in developing and working toward their Annual Service Agreement goals
  • Assist with ADLs, budgeting, healthy living, community access, self-advocacy, and independent-living skills.
  • Provide individualized skills training and teaching strategies to promote independence.
  • Provide timely and accurate documentation in accordance with CSS requirements.
  • Participate in interdisciplinary team meetings to review progress and ongoing support needs.
  • Report concerns, emerging needs, or safety issues promptly to supervisors.
  • Demonstrate flexibility in scheduling to meet client needs and maintain consistent support.
  • Provide transportation in personal vehicle for community-based skill development.

Shared Responsibilities Across Both Roles

  • Adhere to mandated reporting laws for suspected abuse, neglect, or exploitation.
  • Maintain professional boundaries and a positive, client-centered approach.
  • Work independently and as part of a collaborative team across multiple settings.
  • Provide personal care and behavioral support as needed.
  • Problem-solve independently and adapt supports based on individual needs.
  • Maintain strong communication with clients, families, employers, and team members.
  • Other duties as assigned by supervisors of both departments.

Qualifications

Education and Experience

  • High school diploma or equivalent required
  • 1-2 years’ experience in the field of developmental services preferred
  • Experience with writing and reviewing documentation for quality and accuracy in a human services setting preferred
  • Proficiency in Microsoft Office Suite and database systems
  • Valid driver’s license and access to a safe operating vehicle during scheduled working hours

  • Ability to work both independently and collaboratively as part of a team
  • Maintains high attention to detail
  • Strong computer literacy (including but not limited to proficiency with word processing, spreadsheets, email, and database software—MS Office)
  • Ability to multi-task and meet deadlines under very specific timeframes
  • Maintains a high level of organization
  • Ability to communicate effectively (reading, writing, speaking) in English
  • Strong customer services skills
  • Ability to work well under pressure

In an emergency, all staff may be required to perform activities that are above and beyond what is indicated on this job description to ensure the safety of the individuals we serve.

All staff must adhere to the NH state law requiring mandated reporting of suspected abuse, neglect, or exploitation. Additionally, staff are required to maintain criminal, motor vehicle, DCYF, OIG, and Bureau of Elderly and Adult Services record checks that meet agency standards. Staff are also required to travel and transport clients when applicable. This includes having access to a reliable vehicle, a valid driver’s license and maintaining comprehensive personal auto insurance of $100,000/300,000.
