As a Clinical Direct Support Professional, you'll play a vital role in empowering individuals with intellectual and developmental disabilities to build independence, develop life skills, and experience success at home and in their community. This position supports a female model behavioral/medical home with long-term residents, allowing you to build strong, trusting relationships and celebrate meaningful progress over time.

Essential Responsibilities

As a Clinical Direct Support Professional, you will:

  • Follow the daily program schedule and complete assigned responsibilities as directed by your team and Program Manager.
  • Support individuals in achieving their goals by consistently implementing service plans and teaching daily living skills.
  • Ensure all services meet Residential Standards and Regulations established by the New Hampshire Division of Mental Health and Developmental Services and The Moore Center.
  • Complete accurate, timely documentation, including daily notes, reports, and required recordkeeping.
  • Communicate effectively with individuals served, coworkers, supervisors, and other members of the support team.
  • Assist individuals with activities of daily living (ADLs), including personal care, while encouraging independence and self-advocacy.
  • Promote the health, safety, and well-being of individuals in all home and community settings.
  • Safely supervise self-administration of medications or administer medications in accordance with agency policies and training.
  • Complete required behavioral support training and consistently implement approved behavior support and risk management plans.
  • Accompany and support individuals during medical appointments and other community outings.
  • Provide safe transportation using agency-approved vehicles.
  • Participate in required trainings, meetings, and ongoing professional development.
  • Help maintain a clean, safe, and welcoming home environment.
  • Remain on shift until appropriate coverage is available, as directed by the House Manager or Shift Leader.
  • Comply with New Hampshire's mandated reporting requirements for suspected abuse, neglect, or exploitation.
  • Perform other duties as assigned to support the needs of the program and the individuals we serve.

Requirements

Education and Experience:

  • Bachelor's degree in Human Services or a related field, or an equivalent combination of education and experience.
  • Minimum of 1 year of experience in human services required; 3 years preferred.
  • Valid driver's license with access to a safe, reliable vehicle during scheduled work hours.
  • Proof of automobile liability insurance with minimum coverage of $100,000/$300,000.
  • Satisfactory criminal background, motor vehicle, and Bureau of Elderly and Adult Services (BEAS) background checks.
  • Completion of required state training through Relias Learning or an equivalent program preferred.
  • Successful completion of Management of Aggressive Behaviors (MOAB) Part I and Part II training.
  • As a bona fide occupational qualification (BFOQ), Direct Support Professionals must be willing to accompany individuals to religious services based on each person's preferences.

  • Successfully completes medication administration training, if required.
  • Demonstrates flexibility with scheduling and daily responsibilities.
  • Is proficient in Microsoft Office (Word, Excel, Outlook) and comfortable learning new software and technology.
