Job Title: Registered Nurse
Location: St. Louis, MO
Terms: contract parttime
Travel: Up to two hours away from St. Louis. The current case load would include homes off 44, 55, and South St. Louis County
Caseload
The starting caseload would include five with the opportunity to start with 10 individual if preferred and continue to increase as we grow
Agency Overview:
The Momentum Project LLC contracts with the Department of Mental Health and Children's Division to provided host home services to children and adults with developmental disabilities. Our Mission is to serve individuals who have intellectual and developmental disabilities with building momentum in their lives to reach their highest potential.
Role Responsibilities:
- Provide nursing oversight to host homes and ISL homes ensuring all requirement documentation is available and physician orders are being followed
- Provide L1MA and nurse delegation training
- Completes a nursing summary once per month
- Be available by phone to provide nursing direction as needed
- See individuals within in 24 hours of placement and within 24 hours of being discharged from a hospital
- Maintains all required training
- Adheres to the policy and procedures manual
- Other duties may be assigned
Requirements:
- Be 18 years of age or older.
- Maintain a nurse's licenses
- Pass a background screening with the Family Care Safety Registry
Pay: $50.00 per hour
