We are seeking a compassionate, creative, and motivated professional to lead our Transitional Living Program (TLP) while providing direct case management and individualized support to youth ages 16–22 experiencing housing instability. This unique role combines program coordination with meaningful, relationship-based work that empowers young people to build stability, independence, and hope for the future.

What You’ll Do (You’re Day to Day Adventure):

  • Oversee the day-to-day operations of the Transitional Living Program, ensuring high-quality, youth-centered services.
  • Provide strengths-based, trauma-informed case management that values youth voice, choice, and active participation in all aspects of service planning.
  • Support youth in developing independent living skills, including housing stability, education, employment, financial literacy, transportation, health, and overall well-being.
  • Coordinate services and build collaborative partnerships with landlords, schools, employers, healthcare providers, and other community organizations.
  • Maintain timely documentation, case notes, grant reporting, outcome data, and program records in accordance with funding and agency requirements.
  • Represent The Mentor Connector in the community by fostering positive relationships with partners and helping strengthen the Transitional Living Program.
  • Help out wherever needed we’re a team that pitches in and allows us to use our strengths and passion to get the work done.

What Makes You Shine:

  • Bachelor's degree in Human Services, Social Work, Psychology, Behavioral Science, or related field (or equivalent experience).
  • Experience in case management, youth services, housing, or related human services preferred.
  • Knowledge of adolescent and young adult development and trauma-informed care
  • Strong communication, organizational, and time management skills.
  • Ability to work independently while contributing to a collaborative team.
  • Valid driver's license, reliable transportation, and willingness to travel throughout Rutland County.
  • Successful completion of state, FBI fingerprint, and background checks prior to employment.

● Knowledge of issues related to homelessness, substance misuse, or mental health

What You’ll Get:

● A full-time gig with paid vacation, sick time, holidays, and a stipend toward health and wellness (some staff use this for healthcare benefits)

● Salary range: $49,260–$55,000 annually, depending on qualifications and experience.
