We are seeking a compassionate and experienced Primary Care Nurse Practitioner (NP) to provide in-person primary care services within a nursing home and long-term care setting. This role focuses heavily on patient interaction, counseling, education, chronic disease management, and building meaningful relationships with residents and families.
Primary Care Nurse Practitioner Responsibilities
- Spending significant time communicating with patients, families, and caregivers regarding health concerns, goals of care, medications, and treatment options.
- Providing comprehensive primary care to residents in a skilled nursing or long-term care facility.
- Educating residents and families on disease prevention, medication adherence, nutrition, and lifestyle modifications.
- Document encounters accurately in the electronic medical record (EMR).
Primary Care Nurse Practitioner Requirements
- Current Nurse Practitioner license in the applicable state.
- Board certification as Family NP (FNP), Adult-Gerontology NP (AGNP), or related specialty preferred.
- DEA license preferred.
- Experience in geriatrics, long-term care, hospice, primary care, or nursing home settings preferred.
- Strong communication and counseling skills.
- Compassionate bedside manner and patient-centered approach.
- Comfortable working independently while collaborating with interdisciplinary teams.
General Information
- Part Time Compensation: $65.00 - $75.00 per hour.
- 7-8 hours visiting patients plus paperwork.
- Schedule: typically, 9AM – 5PM.
- 15-20 patients per visit.
