Infinite Medical P.C. is seeking a Primary Care Physician with a valid New York State Limited Permit to provide on-site primary care services to residents in skilled nursing and long-term care facilities in the Bronx, NY. This part-time opportunity is ideal for a qualified physician who is committed to delivering compassionate, patient-centered care to the senior population while collaborating with facility staff and interdisciplinary healthcare teams.
Responsibilities
- Provide primary care services to residents in skilled nursing and long-term care facilities.
- Conduct physical examinations and obtain comprehensive medical histories.
- Evaluate, diagnose, and manage acute and chronic medical conditions within the scope of authorized practice.
- Order and interpret diagnostic tests and laboratory results.
- Develop, implement, and update individualized treatment and care plans.
- Prescribe medications and monitor treatment effectiveness, as authorized.
- Collaborate with nursing staff, facility personnel, and interdisciplinary healthcare teams.
- Communicate with residents and families regarding treatment plans, medical conditions, and care goals.
- Maintain accurate, timely, and compliant medical documentation.
- Promote resident safety, dignity, and quality of care.
- Comply with applicable federal and New York State regulations, facility policies, and professional practice requirements.
- Valid New York State Limited Permit to practice medicine, appropriate to the position and practice setting.
- Medical degree from an accredited institution.
- Eligibility to provide services within the scope and limitations of the issued permit.
- Minimum of 6 months of experience in long-term care or a skilled nursing facility preferred.
- Strong clinical assessment, communication, and critical-thinking skills.
- Commitment to compassionate, person-centered care for older adults.
- Ability to collaborate effectively with facility staff and interdisciplinary healthcare teams.
- Ability to complete clinical documentation and use electronic medical records.
