We are seeking a dedicated and qualified Licensed Dietitian-Nutritionist / Registered Dietitian Nutritionist to join our multidisciplinary team. The successful candidate will play a vital role in developing personalized nutrition plans, conducting patient assessments, and supporting recovery-oriented care across various clinical settings, including acute care and long-term care environments. Duties
- Conduct thorough patient assessments to evaluate nutritional needs, medical history, and dietary restrictions.
- Develop individualized care plans that incorporate nutrition therapy for diverse populations including geriatrics and patients requiring tube feeding or specialized diets.
- Provide expert guidance on weight management, dietary modifications, and nutrition education tailored to patient goals.
- Collaborate with healthcare teams to implement nutrition interventions aligned with diagnostic evaluations and treatment objectives.
- Monitor patient progress, adjust care plans as necessary, and document outcomes using electronic medical record (EMR) systems.
- Offer health coaching to promote sustainable lifestyle changes and improve overall health outcomes.
- Supervise dietetic staff and interns, ensuring adherence to best practices in dietetics and patient safety standards.
- Educate patients and staff about nutrition principles relevant to acute care, geriatrics, and long-term care settings.
Requirements
- Valid licensure as a Dietitian-Nutritionist or registration as a Registered Dietitian Nutritionist (RDN).
- Proven experience in hospital or healthcare settings with expertise in acute care, geriatrics, or long-term care environments.
- Strong knowledge of physiology related to nutrition, metabolism, and disease management.
- Experience with dietary department operations, including menu planning and dietary restrictions management.
- Familiarity with EMR systems for documentation and patient record management.
- Demonstrated skills in supervising staff and providing education on nutrition therapy techniques such as tube feeding and care plans.
- Background in health coaching with the ability to motivate patients toward healthier behaviors.
