Social Worker – Long Term Care
Absolut Care of Aurora Park | East Aurora, NY
Pay Range: $18-$21/hr.
Absolut Care of Aurora Park is seeking a Social Worker to join our Long Term Care team in East Aurora, NY. This role is key in supporting residents and families while ensuring compliance with state and federal LTC regulations.
Key Responsibilities:
- Provide psychosocial assessments and ongoing support for LTC residents
- Assist residents and families with care planning, adjustment, and discharge needs
- Coordinate with interdisciplinary team, nursing, admissions, and community resources
- Support care conferences, resident rights, and regulatory documentation
- Advocate for residents’ emotional, social, and psychosocial well-being
Qualifications:
- Degree in Social Work
- Experience in Long Term Care / Skilled Nursing strongly preferred
- Knowledge of NYS DOH and federal LTC regulations
- Strong communication, documentation, and advocacy skills
