Care Connection Companion
We are seeking a compassionate Care Connection Companion to support patients in their rehabilitation journey at The McGuire Group. Your role will be vital in helping patients achieve their care plan goals while collaborating with our therapy team.
Rate: $17.85 - $21.51/hr. (depending on experience)
Shift: Full-Time, 8:00 AM - 4:00 PM, including every other weekend
Key Responsibilities:
- Implement and monitor patient activities in the Care Connections program.
- Assist with functional mobility, range of motion (ROM) exercises, and application of orthotics as recommended.
- Maintain accurate records of patient participation and communicate daily with the therapy team.
- Report any changes in patient condition to the appropriate staff.
- Support patient rights, privacy, and dignity while adhering to HIPAA standards and facility policies.
