Overview:
The McGuire Group is seeking a dedicated Admissions Screener to serve as the frontline liaison between hospital discharge planners, referral sources, and our long-term care and sub-acute rehabilitation facilities. This is a hospital-based role, focused on screening patients for appropriate placement into our skilled nursing communities across Western New York.
Key Responsibilities:

Serve as the point of contact for hospital discharge planners, social workers, and case managers.

Review medical records and patient information to determine appropriate placement within our facilities.

Conduct medical, clinical, and social assessments for prospective admissions.

Communicate with McGuire Group admissions coordinators, nursing leadership, and insurance authorization teams to facilitate timely transitions.

Coordinate patient transfers and transportation logistics when needed.

Maintain accurate documentation of referrals, screenings, and outcomes in tracking systems.

Build and maintain positive relationships with hospital staff and internal facility teams.
Requirements:

Healthcare background required; experience in admissions, case management, discharge planning, or related roles strongly preferred.

Knowledge of long-term care and sub-acute rehab placement criteria.

Strong communication, organizational, and interpersonal skills.

Familiarity with EMR systems and referral portals (e.g., Epic, PCC a plus.)

Must be able to travel between hospital and facility sites as needed.
