Health Services Admissions / Clinic Liaison Job Summary:
This full-time position is responsible for marketing and coordinating admissions for the Allerton Cove Skilled Nursing and Rehabilitation (SNF) and supporting the Admissions in the rest of the campus; Assisted Living Center, Stafford Shores Assisted Living Memory Support and Mayflower Home Health Agency.
The Health Services Admissions / Clinic Liaison is responsible for developing and implementing strategic marketing plans to consistently and continually increase Resident referrals to the Allerton Cove Skilled Nursing and Rehabilitation, Assisted Living Center and Mayflower Home Health.
The Health Services Admissions / Clinic Liaison is a primary point of contact for prospective Residents seeking admission to The Mayflower Allerton Cove Health Center and executes Admission documents with the Resident at the time of admission. The Health Services Admissions / Clinic Liaison is responsible for tracking each Mayflower resident in the hospital and ensure they have a safe path back to the appropriate service line.
Essential Duties & Responsibilities for Health Services Admissions / Clinic Liaison:
1. Census reconciling and reporting.
2. Mix of office and field based between the Mayflower Campus, hospitals and/or other referral sources.
3. Customer service liaison - hospital visits of Mayflower residents.
4. Provides accurate and timely onsite assessments of referrals to determine appropriateness of admission to the Mayflower (SNF, ALF, Independent Living and Home Health)
5. Reviews clinical information, assess payer sources, assist with insurance authorizations, approvals and admit process.
6. Referral development and processing.
7. Room changes- calls and documentation.
8. Room readiness for new admissions.
9. Perform daily check-ins on hospitalized residents and follow up on discharges to home.
10. Perform any other related duties as required or assigned.
EDUCATION & EXPERIENCE
- RN Certification required.
- 3 years or more education or equivalent experience.
