Help Residents Restore Mobility and Independence
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team at a skilled nursing facility in Florida. This role offers the opportunity to make a meaningful impact by helping residents regain strength, mobility, balance, and functional independence following illness, injury, surgery, or hospitalization.
As a Physical Therapist, you will work collaboratively with an interdisciplinary team to develop and implement individualized treatment plans that support optimal resident outcomes and quality of life.
What You'll Do
- Evaluate residents' physical abilities, mobility limitations, strength, balance, and functional needs.
- Develop and implement individualized treatment plans to improve mobility, function, and overall well-being.
- Provide therapeutic exercises, gait training, balance training, transfer training, and pain management interventions.
- Monitor resident progress and modify treatment plans as appropriate.
- Educate residents, families, and caregivers on exercises, mobility techniques, and injury prevention strategies.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
- Maintain accurate, timely, and compliant clinical documentation.
- Participate in care plan meetings and interdisciplinary team discussions.
Qualifications
Required
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited physical therapy program.
- Current Florida Physical Therapist license in good standing.
- Strong assessment, treatment planning, and clinical decision-making skills.
- Excellent communication and interpersonal abilities.
- Commitment to delivering high-quality, resident-centered care.
Preferred
- Experience in a skilled nursing facility, rehabilitation center, post-acute care, or long-term care setting.
- Knowledge of Medicare, PDPM, and rehabilitation regulations.
- Experience using electronic medical record (EMR) systems.
- Bilingual skills are a plus.
