We are currently seeking an experienced MDS Coordinator with previous knowledge of the resident assessment process, which includes individual assessments, MDS assessments, and resident care plans. The MDS Coordinator manages the resources provided to residents and coordinates the entire continuum of care.
The MDS Coordinator will be responsible for:
- Provide accurate documentation of all services to assure accurate reimbursement from all payor sources.
- Ensure that needed resources are available and that quality care is delivered to all residents
- Coordinate developing a written care plan and assessment for each resident and review and revise as appropriate.
This position requires an active Registered Nurse License in the State of Illinois. A minimum of one year of MDS experience in a skilled nursing facility is preferred but not required.
