Your Role as a Social Services Coordinator:
- Provide social services support to residents, addressing psychosocial, emotional, and environmental needCommunicate with residents and families to resolve concerns, provide guidance, and ensure needs are met
- Complete and maintain comprehensive assessments, including social histories, care plans, and progress notes
- Advocate for residents’ rights, ensuring dignity, privacy, and respect at all times
- Collaborate with interdisciplinary teams, including staff, consultants, and healthcare professionals
- Maintain strict confidentiality of resident information in accordance with HIPAA regulations
- Support a calm, caring, and respectful environment for residents and staff
- Participate in care planning and contribute to individualized resident care strategies
- Attend required trainings, in-services, and meetings
What We're Looking For:
- Bachelor’s degree in Social Work or related field from an accredited college or university
- Current Social Worker license in the State of Ohio
- Minimum of 2 years of experience in a healthcare setting preferred
- Strong problem-solving, organizational, and multitasking skills
- Excellent communication and customer service abilities
- Ability to maintain accurate and timely documentation
- Ability to work both independently and collaboratively as part of a team
- Compassionate, calm, and professional demeanor in emotionally demanding situations
- Ability to read, write, and speak English
