Overview
We are seeking an Intensive Case Managers to join our team. The ideal candidate will have a passion for helping individuals navigate challenging situations and access the resources they need to improve their quality of life.
Duties
- Provide comprehensive case management services to clients, including assessing needs, developing care plans, and coordinating services
- Conduct regular client meetings to monitor progress and adjust plans as needed
- Collaborate with internal and external stakeholders to ensure clients receive appropriate support
- Utilize motivational interviewing techniques to empower clients in setting and achieving goals
- Assist clients in accessing workforce development programs, addiction counseling, and behavioral health services
- Manage crisis situations effectively and provide appropriate interventions
- Maintain accurate and up-to-date client records in compliance with regulations
- Other duties as assigned.
Qualifications
- Bachelor's degree in Social Work, Psychology, Criminal Justice, Political Science, Education, History, Counseling or related field. Applicants may also have a high school diploma with 12 credit hours in social sciences and 2 years in human services with 1 year direct client contact.
- Strong understanding of crisis management techniques
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Valid driver's license, use of car during work hours, and valid auto insurance

Pay: $43,000.00 - $44,000.00 per year
