General Description: Provide intensive case management services to special needs populations to increase client independence, and encourage stability in their housing, and ives.
Essential Functions:
1. Outreach and indentify client needs, including eligibility for services.
2. Set goals necessary to achieve stability, in cooperation with client.
3. Link with existing social services, community supports, housing opportunities/options, and all resources necessary for independence and stability.
4. Monitor service delivery through home visits, community appointments, and contact with other providers.
5. Advocate for clients with other service providers and coordinate services in place.
6. Maintain accurate records.
7. Other duties, as assigned.
