- Act as a dedicated advocate for patients by listening to their concerns, understanding their needs, and providing personalized support throughout their healthcare journey
- Coordinate with multidisciplinary teams to facilitate discharge planning, case management, and access to community resources
- Assist patients in navigating hospital services, including social work support, behavioral health resources, and hospice care options
- Educate patients about medical terminology, treatment plans, and available options to promote informed decision-making
- Support intake processes and conduct assessments to identify patient needs related to acute care, ICU, PICU, or Level I/II trauma center experiences
- Ensure compliance with HIPAA regulations by maintaining confidentiality and handling sensitive information securely
- Utilize electronic health record (EHR) and electronic medical record (EMR) systems efficiently to document interactions and coordinate care plans
- Collaborate with utilization review and management teams to optimize patient care pathways and resource utilization
- Assist with discharge planning by coordinating follow-up appointments, home care services, and managed care arrangements
- Provide emotional support and advocacy for patients dealing with complex health conditions across hospital medicine settings
Experience
- Prior experience in critical care environments such as ICU or PICU is highly desirable
- Strong background in hospital settings with familiarity in emergency medicine or trauma centers (Level I/II) preferred
- Knowledge of medical coding systems including CPT coding, ICD-9/10 coding essential for understanding billing and documentation processes
- Experience working with social work teams or behavioral health services is advantageous
- Proficiency in EMR/EHR systems along with a solid understanding of HIPAA compliance standards
- Background in discharge planning, case management, or utilization management within acute or managed care settings preferred
- Demonstrated ability to communicate effectively with diverse patient populations across pediatric and adult care environments
