What are the responsibilities and job description for the Case Manager position at The League for People with Disabilities?
Highlights of Case Manager
As the Case Manager you will utilize a person-centered approach to write Service Implementation Plans (SIP) by establishing meaningful, measurable, and individualized goals for participants receiving Meaningful Day, Employment, Personal Supports and Behavioral Support Services. The Case Manager will also monitor progress towards goals by completing quarterly goal reviews and maintaining ongoing communication with the DDA Coordination of Community Service (CCS), Direct Service Professionals (DSP), and all other team members.
Essential Responsibilities
Coordinate with DDA CCS’s to schedule initial, annual, and revised Person-Centered Plan (PCP) meetings with all relevant team members.
Provide quality case management services to meet participant needs as they arise, including providing community resources.
Conduct monthly communication with participant and caregivers regarding participant progress and needs.
Serve as the primary Incident Report writer for the department, adhering to reporting timelines.
Attend resource fairs to promote The League’s programs and services.
Requirements
High School Diploma with at least 3 years in case management or previous work with individuals with disabilities or Bachelor's Degree in Social Work, Nursing / Health Science, Human Services, Public Health / Public Administration, or related field
