Overview
This nurse provides, supervises, and coordinates care to support residents' health, safety and independence
Responsibilities
Medication management: Administering, supervising, or assisting with medication regimens to ensure compliance
Health Assessment & Monitoring: Conducting routine wellness checks, monitoring vital signs, tracking changes in physical/cognitive healt, and evaluating for fall risks
Care Planning: Developing and updating individualized care pland in collaboration with physicians and families.
Personal Care and Support: Assisting with, or overseeing the assitance of daily activities such as bathing, dressing, and grooming
Coordination of Care: Acting as a liason between residents, families, physicians, and external healthcare providers to manage appointments and medical needs
Supervision & Education: Managing care staff, educating residents on chronic disease management, and implementing wellness and exercise prgrams.
Documentation: Maintaining detailed, accurate medical records of resident's health status, treatments, andincidents.
Emergency response: Providing on-call or immediate, on site nursing care during emergencies.
Experience
- Valid LPN license with current state licensure required; additional certifications in Basic Life Support (BLS).
- Proficiency with medical terminology, and medical documentation standards.
- Experience working with individuals with physical and psychiatric disabilities or developmental delays in community or home care environments is desirable
