Location: Trumbull and Stamford, CT
Job Type: Full-Time / Contract
Shift/Schedule: Monday – Friday, 8:00 AM – 4:30 PM (No on-call or weekend requirements)
Weekly Hours: 40 Hours Guaranteed
Productivity Expectations: 30 points
Mileage Reimbursement: Mileage is reimbursed for travel from the second patient of the day to the last patient.
Pay Rate: $48/hr

- Case Management: Manage an assigned patient caseload, ensuring coordinated, continuous, and high-quality care in partnership with the interdisciplinary team.
- Admissions & Recertifications: Conduct comprehensive admission assessments and process recertification paperwork accurately and efficiently.
- RN Evaluations: Perform thorough in-home evaluations to establish patient needs, functional status, and individualized plans of care.
- Wound Care: Provide skilled wound care interventions, monitor healing progress, and document clinical outcomes.
- EMR Documentation: Accurately document all clinical notes and patient data using the Homecare Homebase (HCHB) and OASIS systems.
- Previous homecare experience utilizing the Homecare Homebase (HCHB) platform.
- Hands-on experience completing OASIS assessments.
