Registered Nurse (RN) - Home Health Case Manager

Location: Trumbull and Stamford, CT

Job Type: Full-Time / Contract

Shift/Schedule: Monday – Friday, 8:00 AM – 4:30 PM (No on-call or weekend requirements)

Weekly Hours: 40 Hours Guaranteed

Productivity Expectations: 30 points

Mileage Reimbursement: Mileage is reimbursed for travel from the second patient of the day to the last patient.

Pay Rate: $48/hr

Job Overview

We are seeking a dedicated and experienced Registered Nurse to manage a caseload in the Trumbull and Stamford areas. In this role, you will deliver high-quality, patient-centered care directly in the home setting. You will be responsible for comprehensive care delivery, ranging from initial RN evaluations and admissions to recertifications, wound care, and ongoing case management.

Key Responsibilities

• Case Management: Manage an assigned patient caseload, ensuring coordinated, continuous, and high-quality care in partnership with the interdisciplinary team.
• Admissions & Recertifications: Conduct comprehensive admission assessments and process recertification paperwork accurately and efficiently.
• RN Evaluations: Perform thorough in-home evaluations to establish patient needs, functional status, and individualized plans of care.
• Wound Care: Provide skilled wound care interventions, monitor healing progress, and document clinical outcomes.
• EMR Documentation: Accurately document all clinical notes and patient data using the Homecare Homebase (HCHB) and OASIS systems.
  
  

Required Qualifications

• Previous homecare experience utilizing the Homecare Homebase (HCHB) platform.
• Hands-on experience completing OASIS assessments.
