Seeking an experienced RN Case Manager to support acute care case management, focusing on patient care coordination, discharge planning, and regulatory compliance in a hospital setting.

• nEnsure CMS compliance, including admission reviews, continued stay, IMM & MOO
• NCoordinate transitions of care and post-acute placement
• sMaintain accurate and timely clinical documentatio
• nCollaborate with physicians, nursing, and interdisciplinary team
• sIdentify high-risk patients and prevent readmission
• sSupport audits, compliance, and regulatory standard
