Registered Nurse

Nurse (RN) – Contract | On‑Site | Valparaiso, IN

Location: Valparaiso, IN (covering Lake, LaPorte, Starke & Marshall Counties)

Employment Type: Contract

Worksite Type: Fully On‑Site

Weekly Hours: 30–40 guaranteed

Shift: Monday–Friday, 8:00 AM–4:30 PM

On‑Call: Rotating weekend on‑call

Mileage Reimbursement: Yes (from first patient to last patient)

About The Role

We are seeking an experienced Nurse (RN) to provide high‑quality home‑health care across multiple counties in northern Indiana. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently in the field.

Key Responsibilities

• Conduct comprehensive patient assessments in home‑based settings
• Communicate effectively with physicians, interdisciplinary teams, and internal staff
• Participate in weekly team meetings
• Receive, clarify, and implement medical orders
• Meet productivity expectations of 25–30 points per week
  
  

Required Qualifications

• Associate Degree in Nursing or higher
• Active RN license (state‑appropriate)
• BLS certification
• Ability to complete required occupational health screenings
• Reliable transportation for field visits
  
  

Preferred Qualifications

• Experience with Homecare Homebase (HCHB)
• OASIS documentation experience or training
• Strong organizational and communication skills
