Primary Responsibilities
- Conduct comprehensive clinical assessments
- Manage acute and chronic medical and behavioral health conditions
- Ensure accurate and complete documentation, including ICD‑10 coding
- Collaborate with the interdisciplinary care team
- Lead advanced illness and care‑planning discussions
- Provide education and counseling to patients and caregivers
- Active, unrestricted state license (or ability to obtain by start date)
- NP: Graduate of an accredited MSN or DNP program
- PA: Graduate of an accredited Physician Assistant program
- Active national certification:
- NP: AANP or ANCC (FNP, ANP, GNP, AGACNP)
- PA: NCCPA certification
- Active DEA licensure/prescriptive authority
- Ability to meet physical job requirements (lifting 30‑lb bag, navigating stairs, varied environments, ability to sit/stand/kneel for assessments)
- Reliable transportation
- Ability to obtain a collaborative practice agreement (if required by state)
- Full COVID‑19 vaccination (per federal, state, local, and client requirements)
- Must pass pre‑employment drug screening (UnitedHealth Group is a drug‑free workplace)
- 1 year of clinical experience (long‑term care preferred)
- Experience supporting patients with complex behavioral, social, or functional needs
- Knowledge of geriatrics and chronic illness management
- Experience with advanced illness and end‑of‑life discussions
- Proficiency with electronic and written medical documentation
- Shift: 8:00 AM – 5:00 PM
- Days: Monday–Friday
