Schedule: Full‑time, 40 hours/week

The Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) will support the Senior Community Care (SCC) program, a longitudinal, integrated care model designed to coordinate and deliver clinical services to high‑risk members in facility-based settings. This role is part of an interdisciplinary care team focused on improving outcomes for complex, vulnerable populations.

Primary Responsibilities
- Comprehensive Assessments — Perform full clinical evaluations for ISNP members
- Acute & Chronic Condition Management — Provide ongoing medical and behavioral health management
- ICD‑10 Documentation — Ensure accurate, complete, and compliant coding
- Interdisciplinary Collaboration — Communicate effectively with the care team to coordinate services
- Advanced Illness & Care Planning — Lead discussions around goals of care and treatment preferences
- Patient & Caregiver Education — Provide counseling, disease education, and support
- Active, unrestricted NP or PA license in your state of residence (or ability to obtain by start date)
- NP: Graduate of an accredited MSN or DNP program
- PA: Graduate of an accredited Physician Assistant program
- Active national board certification:
- NP: AANP or ANCC (FNP, ANP, GNP, AGACNP)
- PA: NCCPA
- Current DEA licensure/prescriptive authority
- Ability to meet physical job requirements:
- Lift a 30‑lb medical bag in/out of vehicle
- Navigate stairs and varied home/facility environments
- Sit, stand, kneel for physical assessments
- Reliable transportation
- Ability to obtain a collaborative practice agreement (if required by state)
- Full COVID‑19 vaccination (per federal, state, client requirements)
- Ability to pass pre‑employment drug screening (UnitedHealth Group is a drug‑free workplace)
- 1 year of clinical practice experience (long‑term care preferred)
- Experience caring for patients with complex medical, behavioral, or social needs
- Knowledge of geriatrics, chronic illness, and advanced illness/end‑of‑life care
- Proficiency with electronic medical records and digital documentation
