Nurse Practitioner / Physician Assistant – Workplace Primary Care Clinics
Schedule & Locations (36 hours/week)
Provide high‑quality primary and episodic care to Mohawk Industries employees and dependents in a workplace‑based clinic setting. Support physicians and other medical providers in assessing, planning, and delivering patient care.
Key Responsibilities
Schedule & Locations (36 hours/week)
- Tuesday & Wednesday: 6:00 AM – 6:00 PM - Rome, GA 30165
- Thursday: 6:00 AM – 6:00 PM - Summerville, GA 30747
Provide high‑quality primary and episodic care to Mohawk Industries employees and dependents in a workplace‑based clinic setting. Support physicians and other medical providers in assessing, planning, and delivering patient care.
Key Responsibilities
- Provide primary care in an onsite employer clinic
- Deliver episodic/low‑acuity urgent care
- Manage extended episodic acute care (typically 4–5 visits for short‑term conditions)
- Provide chronic condition education and co‑management with external PCPs
- Perform and oversee CLIA‑waived testing
- Dispense prescription medications as allowed
- Oversee specimen collection processes
- Support biometric screenings and wellness education
- Provide one‑on‑one health coaching and refer to wellness programs
- Partner with HR/Benefits teams to improve employee health
- Collaborate with and potentially supervise clinic support staff (MA, LPN, etc.)
- Certified Family Nurse Practitioner
- Active Georgia NP License
- 3 years as a CRNP in family practice or internal medicine
- Strong primary care experience, including women’s health
- Active DEA
- Active BLS
- Phlebotomy experience preferred
- Supervisory experience preferred
- EHR experience; EPIC a plus
