Top Responsibilities
- Comprehensive assessments — Perform full head‑to‑toe evaluations and ongoing patient monitoring.
- Wound care — Manage complex wounds, including wound vac therapy.
- IV therapy — Administer and monitor IV medications and therapies.
- Enteral feeding management — Provide care for patients with feeding tubes.
- Urinary drainage device management — Maintain and troubleshoot urinary catheters and drainage systems.
- Care coordination — Collaborate with physicians, therapists, and caregivers to ensure seamless care.
- Disease management education — Teach patients and families about chronic condition management.
- Medication reconciliation — Ensure accuracy and safety of all prescribed medications.
- Case management — Oversee the full plan of care, ensuring regulatory compliance and timely documentation.
- OASIS documentation — Complete accurate, compliant OASIS assessments.
- Active, unencumbered RN license in the state of Maryland
- Current CPR certification
- Prior home health experience
- Proficiency in performing OASIS assessments
- Strong clinical assessment and critical‑thinking skills
- Reliable transportation and ability to travel within assigned territory
- OASIS certification
- Wound care certification
- Experience with advanced wound modalities
- Strong communication and patient‑education skills
- Ability to work independently with minimal supervision
Salary : $49 - $50
