Job Summary
The Case Manager RN plays a key role in coordinating patient care, assessing clinical needs, and supporting care plans in collaboration with the interdisciplinary team. This position requires strong clinical knowledge, excellent communication skills, and the ability to work efficiently within a digital, Windows based care environment.
Top Responsibilities
- Conduct comprehensive patient assessments to identify medical, functional, and psychosocial needs
- Develop, implement, and monitor individualized care plans
- Coordinate care across providers, specialists, and community resources
- Facilitate patient education related to treatment plans, medications, and ongoing care
- Document all patient interactions, care plans, and assessments accurately in electronic systems
- Collaborate with physicians, nurses, and care teams to ensure optimal patient outcomes
- Support transitional care needs and discharge planning as required
- Follow compliance standards, safety protocols, and documentation requirements
Required Qualifications
- Current, unrestricted RN license in the state of Florida
- 3 years of clinical experience in a hospital, acute care, or direct care setting
- Proficiency in typing and navigating within a Windows based environment
- Minimum Education: Associate Degree
