PURPOSE:

The Preventive Services Department provides child welfare services using Evidence Based models to ensure that families are able to provide safe environments for their children, keep children safe in their homes and reduce the need for foster care. Prevention is an essential part of the child welfare continuum and requires a commitment to the wellbeing of all members of a client family. All services are provided in a family’s home and community. 

POSITION OVERVIEW:

   The Case Associate functions as a Case Planner and is responsible for supplying appropriate interventions to ensure client care. Working within the client’s community, develops action plans effectively identify and utilize existing resources and establish new resources for client care.

KEY ESSENTIAL FUNCTIONS:

  • Routine community contacts on behalf of social worker, e.g. school reports, medical reports. Locates and makes use of appropriate community resources for clients.
  • Evaluates actual living conditions of clients through home visits.
  • May interpret agency function to client and demonstrates this in terms of concrete help Provides structure to the client, evaluates and assess usage of independent living skills and provides guidance in these areas to the client. May run groups, under the supervision of the social worker.
  • In concert with the Sr. Case Planner, evaluates clients for the purpose of beginning, monitoring and following up on individualized treatment programs including child safety.
  • Individualizes and understands the needs of the individual client, the family and its support network in relation to each other
  • Identifies appropriate treatment or services providers and facilitates client referral to internal and external and/or community linkage programs
  • Performs individual/group counseling; long or short-term depending on client needs
  • Conduct home visits in which the needs of the family and child safety assessed
  • Identify help for clients in obtaining needed services. Helping clients and families maintain medical, financial and social resources would be an example of such services
  • Provides case management to clients
  • Identifies and provides access to specific appropriate concrete services when necessary or requested and are in the best interest of the client
  • Performs Community outreach and establishes linkages with other groups, individuals or organizations which may be helpful to our clients
  • Provide all necessary notes, progress notes, risk Assess
  • Make report to State Central Registry when required.
  • Identify and monitor high risk families on caseload who are at high risk of abuse, neglect and further system involvement.
  • Maintain the case records consistent with JBFCS and ACS standards.
  • Work a hybrid schedule based on program needs
  • Complete agency internal Incident Reports in Alltrac system.
  • Participate in Family Team Conferences (FTCs).
  • Other duties as assigned

   CORE COMPETENCIES for the position include:

  • Basic social work knowledge and understanding of stages of change
  • Ability to provide in-home child welfare services
  • Ability to establish rapport and therapeutic relationship with families
  • Understanding of the use of family therapy principals to provide services (preferred)
  • Understanding of early childhood development and related theories and models of practice (preferred)
  • Strong Assessment skills to evaluate dynamics and interactions within the family (preferred)

EDUCATIONAL/TRAINING REQUIRED:

  • BA Degree, AA Degree with some experience in a community or social group, or High School Diploma with at least five (5) years of experience as a case assistant in a community or social service group, etc.

EXPERIENCE REQUIRED/LANGUAGE PREFERENCE:

  • Bilingual (preferred)

COMPUTER SKILLS REQUIRED:

  • Knowledge of Microsoft office required
  • Knowledge and experience with the Connections system preferred

VISUAL AND MANUAL DEXIERITY:

  • Limited applications of manual dexterity and hand-eye coordination.

WORK ENVIRONMENT/PHYSICAL EFFORT:

  • To perform the essential functions of this job the candidate must be able to travel within New York City carrying equipment such as a notebook, forms, laptop, mobile hotspot and cell phone weighing up to approximately 10 pounds
  • Risks/hazards associated with the position are those which may be encountered travelling around New York City and interfacing with clients experiencing an acute mental health crisis or serious emotional disturbance
  • Travel in the community and home-based work.
  • Position is currently hybrid but can become 100% in person based on program need.
