What You’ll Do:
• Coordinate Personalized Plans: Develop and implement individualized plans for each client, including Staff Action Plans, Individual Plans of Protective Oversight (IPOP), Personal Expenditure Plans, Money Management Assessments, and Levels of Supervision.
•Lead Interdisciplinary Teams (IDT): Facilitate team meetings to ensure that clients receive quality active treatment and that their needs are met holistically.
•Monitor Progress: Track and evaluate the progress of clients’ goals and achievements, adjusting plans as necessary.
•Ensure Compliance: Maintain accurate, up-to-date electronic records and ensure all documentation is in compliance with OPWDD (Office for People With Developmental Disabilities) regulations.
•Support Independence: Assist clients in gaining independence with life skills, social interactions, and other personal goals.
•Provide Input for Training Programs: Help develop and implement training programs for staff and clients, monitoring progress and outcomes.
Qualifications:
Educational Requirements
Bachelor’s degree in a Human Services field (or related field) required.
Experience & Skills:
• 1 years of relevant experience in a similar role preferred.
•Proficiency in person-centered planning, instructional methods, and progress tracking. Strong organizational, communication, and interpersonal skills.
• Valid New York State driver’s license is required.
• Understanding of OPWDD regulations and the ability to maintain accurate documentation.

Schedule: Full-Time 40 hours per week
