Resident Care Coordinator (Full-Time 40 Hour Medication Aide, Medical Assistant, or LPN)About the role
We are seeking a dedicated and experienced Resident Care Coordinator to join our team. This individual must be a 40 Hour Medication Aide, Medical Assistant, or LPN who will take a leadership role in overseeing resident care, supporting staff, and ensuring daily routines are carried out with consistency and compassion.
Key Responsibilities
- Lead and direct daily resident care, ensuring all cares and tasks are completed in a timely, organized routine.
- Act as a role model and point of support for caregivers and Medication Aides on shift.
- Communicate effectively and professionally with doctors, pharmacies, outside services and families.
- Respond to staff calls and provide guidance on resident concerns, needs, or change in condition.
- Assist with some staffing-related calls and covering the shifts or filling in as needed.
- Promote teamwork and ensure a positive, resident centered environment.
Qualifications
- Must be a current 40 Hour Medication Aide, Medical Assistant, or LPN
- Previous experience in assisted living, memory care, or long-term care is strongly preferred
- Strong leadership and organizational skills
- Excellent communication and interpersonal skills for interaction with residents, families, staff, and healthcare providers
- Ability to remain calm, flexible, and solution-focused in a fast-paced care environment
Schedules and Benefits
- Full-time Monday-Friday 8:00 am- 4:00 pm
