This is an exciting opportunity to join a smaller skilled nursing facility with lower patient ratios, allowing our nurses to provide more personalized, attentive care to our residents while working alongside leadership that values and supports its staff.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
