What are the responsibilities and job description for the CAMPUS MEDICAL TRANSPORT position at The House of The Good Shepherd?
Basic Function/Overall Responsibilities: To provide support and safe transportation for clients of the agency to and from medical/ clinical, educational, home visits and dental appointments in accordance with the agency's vehicle policy.
Principal Assigned Responsibilities:
- Coordinate all medical appointments and prepare necessary paperwork for such appointments.
- Transport clients to and from medical/dental appointments to include out of town appointments. Provide transportation to educational and home visits as scheduled.
- Coordinate follow-up appointments as necessary, collect provider's notes; make necessary copies to give agency providers and nurses assigned to the client.
- Provide support supervision to youth with medical needs.
- Organize and keep appointments logbook up-to-date.
- Pick up medical equipment for client's post appointments to include but not limited to eyeglasses, hearing aids, etc.
- Complete all required agency trainings.
- Maintain TCI certification.
- Maintain Med Certification.
- Other tasks as requested by supervisor(s).
Education and Experience Requirements:
l. Clean driving abstract. Must have three years of driving experience.
2. High school diploma or GED.
3. Preferred experience with children with behavioral/psychiatric issues.

7. Asks age, developmentally appropriated questions, and actively listens to children's responses.
8. Valid New York State driving license.
