Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team on a Per Diem basis!
Schedule: 2 days per week 8:30am-4:30pm

Duties & Responsibilities
- Provide assessment of newly referred patients and assess health needs in home care setting.
- Work with patient to create a patient centered plan of care.
- Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family.
- Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.
- Provide supervision and training of PCA/HHA staff.
Qualifications
- NYS Licensed Registered Professional Nurse.
- Minimum of Associate's Degree required.
- One year Nursing experience required.
- Home care experience a plus.
